Job Description

Entry-Level Social Worker / Case Manager

Location: Rochester, NY
Compensation: Firm $21.50/hour + $1,000 first-year retention bonus (paid quarterly)
Schedule: Monday - Friday (Flexible start time)
Travel: 50% fieldwork, 50% office-based; mileage reimbursed.

About the Role

As a  Social Worker / Case Manager , you will work with clients who face significant challenges, including mental health issues, substance use, and housing instability. You’ll play a pivotal role in empowering clients to achieve permanent supportive housing and financial independence while enjoying the support of a collaborative and experienced team.

What You'll Do

Client Support & Advocacy:

  • Develop individualized plans to help clients achieve self-sufficiency and permanent housing.
  • Conduct home visits, ensuring clients’ homes are safe and advocating for their needs with landlords.
  • Provide crisis intervention and harm reduction services to clients actively using substances or in urgent situations.
  • Assist with navigating and securing entitlements (e.g., DHS, SSA benefits) and other community resources.

Relationship Building & Case Management:

  • Foster trusting relationships with clients while maintaining professional boundaries.
  • Monitor progress and adjust action plans to meet goals.
  • Advocate for clients’ well-being and independence in collaboration with other service providers.
  • Educate clients on life skills, including nutrition, hygiene, and financial management.

Team Collaboration:

  • Work closely with a supportive and experienced team that values shared success and collaboration.
  • Shadow experienced staff and supervisors to learn best practices and program expectations.
  • Attend monthly community meetings to stay updated on resources and build your professional network.

Administrative Tasks:

  • Document case notes and submit reports to meet grant funding requirements.
  • Manage client intakes and eligibility determinations using an easy-to-learn database system.
  • Coordinate financial resources, including rent payments, credit card receipts, and check requests.

What We're Looking For

  • Education: An associate degree in human services (or related field) with field experience; a bachelor’s degree in social work, psychology, sociology, or criminal justice is preferred.
  • Experience: 0-2 years of experience in social work, case management, or related fields. Training or exposure to trauma-informed care, mental health, and substance abuse is highly valued.
  • Skills: Strong communication, organization, and problem-solving skills.
  • Requirements:
    • Valid driver’s license and reliable transportation.
    • A genuine passion for helping individuals overcome barriers to stability.
