THE POSITION: 

• Cases Close to Home 
• Hours: May include days, evenings, nights and weekends. Full time, part time and PRN shifts available 
• Case Details: Various ages of children ages birth thru 30+years old with technologies that might include g-tube, trach and ventilator. 
• Now Offering up to a $2,000 Sign On Bonus for Overnights and Weekends!

Responsibilities

• Assists in developing, reviewing, and revising the plan of care.
• Provides nursing care as authorized by client's payer source, in accordance with physician's plan of treatment and individual plan of care.
• Observes, records, and reports reactions to treatment and any changes in client's condition to appropriate personnel and/or physician.
• Provides ongoing instruction and supervision of client, family, or other health team personnel in the plan of treatment.
• Monitors and records patient's condition and documents provided care services via electronic charting.
• Participates in education programs, quality assessment activities, nurse's meetings, and other staff meetings as required.

Requirements

• Active Licensed Practical Nurse or Registered Nurse License
• Problem solving skills and ability to multi-task
• Excellent Communication Skills
• Positive, Professional attitude
