Role: Dietitian/ Dietitians and Nutritionists

Duration: Full-time (40 hrs/week)

Location: Delano, CA 93215

Required

• Bachelor's degree or higher
• Dietitian (1+ years)
• Valid Registered Dietitian Nutritionist (RDN) in the state of California (CA)
• Registered by the Academy of Nutrition and Dietetics on the Commission of Dietetics Registry? (yes)

Preferred

• 1+ years of experience in ALL of the following:

• ESRD
• Dialysis

• Member of the American Dietetic Association (yes)
• As an In-Center Dietitian, you will be responsible for building trusting relationships with patients and their families, providing patient-specific nutritional plans, and ensuring ongoing nutritional support.
• Your role will directly impact patients' quality of life by utilizing dietary education and planning.

Key Responsibilities:

• Complies with all company policies and procedures as well as federal, state, OSHA, and other agency regulations applicable to the position
• Handles patient information with discretion and sensitivity and maintains HIPAA compliance
• Utilizes tactful and professional communication to promote positive public relations with patients, patient family members, physicians, co-workers, outside agencies, and service providers
• Ensures that patients receive appropriate care related to renal diet and medications from Long Term Care Facilities
• Provides comprehensive nutritional assessments for dialysis patients, ongoing instruction and counseling, monitors adherence to diet, provides nutritional educational material, and reviews monthly lab work
• Assists with obtaining IDPN, nutritional resources, and protocols
• Participates in the completion of Comprehensive Assessments and Plans of Care for all patients as a member of the Interdisciplinary Team (IDT)
• Participates in the facility's Continuous Quality Improvement process as a member of the Quality Assessment Performance Improvement (QAPI) team
• Participates in national and state programs on renal nutritional services and keeps abreast of changes
• Reviews monthly lab work on each patient and makes changes in medications and diet orders in conjunction with the attending physician and/or protocols and in accordance with state-specific regulations
• Refers patients for assistance with nutritional resources such as financial assistance, community resources, or in-home assistance and helps in getting nutritional supplements as needed
• Performs job duties in accordance with all state/licensing requirements
• Assists with other projects and assignments as requested by Clinic Leadership

Schedule and Location Details

• M-F, 8:00-4:30p - Saturdays if needed.
• Candidates will provide services between Delano, CA and Bakersfield, CA.
